Dear patient:
Welcome to my practice. This is a concierge practice model, and for those of you who are unfamiliar with this model, it is a highly personalized approach that will be limited to a fewer number of patients to allow for a more comprehensive approach to the patient. A lot of the constraints in medicine that we now have are basically eliminated or markedly reduced with this model since it allows for more time with the patient and more access to the physician.
The practice will be one of a holistic approach to your care, including comprehensive exams, preventative medicine (i.e. well-care) and I will even be attending to your care when you are admitted to the hospital or a rehab facility.
As part of the service, I will be offering the following:

24/7 access to my services via my office phone and/or email or texting. My personal cell phone will also be available if needed.

Guaranteed same day sick visits Monday through Friday and even availability on weekends if need be.

Very little wait time after arrival to the office.

If, on rare occasions, I am not able to be available, you will be notified of this ahead of time, and all arrangements will be made to expedite your medical care.

Hospital care will be provided by myself if you happen to be admitted to the hospital.

If you are admitted to a hospital that I do not attend to, I will contact the hospitalist attending to your care and be updated on your care regularly.

If you go to a nursing facility for rehab, etc., I will also be attending to your care there, as well.

Expediting coordination of care with consultants in a timely and efficient manner.

Filling out any forms that you will need for prior authorization for medications, time off work, FMLA, Home healthcare services,etc.

Prescription medication refills will be handled on a same day basis.
Should you ever have any questions about these services or any of your medical care, of course, I will be readily available.

HEALTHCARE MANAGEMENT
Through the concierge model I am able to provide comprehensive care management to my patients. This may include developing a preventative care plan, dealing with acute healthcare concerns as they arise and coordinating care from specialists. As your primary care provider, I help my patients navigate all their healthcare decisions.
ROUTINE CHECKUPS
I view each patient as a whole rather than a single symptom to be treated. Your health deserves proper care and attention and my focus is to make sure all your healthcare questions and concerns are addressed. I’m dedicated to building long-lasting relationships based on trust and medical integrity with every single patient.
ACUTE CARE
For those times when acute situations arise, I am there to help make sure you receive the highest level of care. I am able to visit patients in the hospital as needed and can make referrals to a range of specialists to address acute or chronic health conditions.
HEALTHCARE MANAGEMENT

PREVENTATIVE CARE
We will work together to develop a preventative care plan that fits your lifestyle. This may include nutrition, exercise and chronic care management. My focus is on keeping patients healthy, not just on treating sick patients.
